Carlton-on-Trent (not "Carleton") is a village in Nottinghamshire, which is a county (this is displayed correctly in the place of death).
Nottingham is a city in Nottinghamshire but this is probably superfluous as "Carlton-on-Trent" should be in the City field and "Nottinghamshire" in the county field.
England in both the State and Country fields is overkill. It seems to be convention for pre-UK (before 1801) to put England in the State field only which stops the full location rendering as United Kingdom which of course didn't exist at the time.
