Dear Researchers:
There are several Horowitz families represented in the Divenishker Vereins (i.e. Dieveniskes) of Mt. Zion in Queens, NY and Beth David in Elmont, NY whose death certificates I am unable to find in the New York City or State Indexes. The following list shows what I know. I have images of the headstones.
1) Mashe Horwitz, Location: Beth David, DOD: 09 Jan 1941, Age: 77, Father: Haim, Described as 'Our Beloved Mother', Comment: most likely Horwitz is the married surname
2) Heshe 'Helen' Horowitz, Locaiton: Beth David, DOD: 09 Feb 1929, Age: 32, Father: Ysroel, Described as: Out Beloved Sister;. Comment: most likely Horowitz is the birth surname

Avraham Eliezer HaLevi, presumed husband; both stones are in a Hurwitz area and there is a bench there inscribed 'Levey', possibly the HaLevi Levey and HaLevi Hurwitz families are branches from same Levitic trunk
