His DN indicates his place of death as the farm Holfontein, Heilbron, Orange Free State Province, with no mention of the Modder River area or Bloemfontein in this record -- with the only exception that his marriage was in Bloemfontein (presumably the home town of his bride, as the custom was at the time). Also,his close relatives are nearly all from the Heilbron district. Maybe his place of residence (Modder River, Bloemfontein) is a mistake?
I know that it has been a long time since that info was posted to this profile and that it may be difficult to find the source again by now, if you can find anything to disprove my theory I should be grateful to see/hear about it.
